Required Documents
- Letter from the principal applicant stating intent to bring family members (prepared by our company / signature only) 1 copy
- IM12 Application Form (prepared by our company / signature only) 1 copy
- All pages of the applicant’s passport *Must be valid for at least 24 months
- Passport-sized color photo 1 copy (blue background)
- Original marriage certificate (in English) 1 copy *For spouse applications
― Obtain an English certificate from the Japanese Embassy by presenting the original family register (issued within the last 3 months) - Birth certificate (in English) 1 copy *For child applications
― Obtain an English certificate from the Japanese Embassy by presenting the original family register (no expiration date) - Certificate of No Criminal Record (if 21 years of age or older)
- Medical Examination Form MFI
- Copy of every PVIP visa page held by the Principal Applicant (one copy per visa)
- Original and copy of the Principal Applicant’s Approval Letter (if lost, a police report is required)
- Certificate letter from the bank for PVIP purposes
- Bank statements submitted for PVIP (must be certified)
- Principal’s income certificate
- Transaction statements verifying the principal’s income
- Principal’s certificate of employment
- Personal bond (prepared by our company)

Step 1: Application Procedures with the Immigration Bureau
1. Once all required documents for the accompanying person’s application are gathered, accompany the applicant to the bank to obtain a bank letter and have copies of fixed-term certificates certified by the bank.
2. After Cosmos verifies the required documents, submit the online application to the Immigration Bureau (processing takes approximately 3-4 months).
3. After the Immigration Bureau approves the renewal, Cosmos will contact the applicant.
4. Pay the accompanying person’s processing fee and visa fee to the Immigration Bureau within 60 days after the Approval Letter is issued.
*The Approval Letter is valid for 6 months. Please complete the issuance procedures within this 6-month period.
Step 2: Visa Issuance Procedures (After Obtaining Approval Letter)

Day 1: Submit required documents to Cosmos → We will accompany you for a simple health checkup (approx. 30 minutes)

(If you entered via an automated counter, please submit your boarding pass and flight reservation confirmation)
• Insurance policy (Not required for those aged 60 or older. If not enrolled, we can arrange it for you)
Day 2: Our staff will handle the procedures at the Immigration Department on your behalf
Days 3 & 4: Reserve days
Day 5: Passport return

Spouse: RM30,000 + 8%
Children and Parents: RM15,000 + 8%
Immigration Department Processing Fee
20-year application: RM100,000/person
10-year application: RM50,000/person
Other Fees
• Visa Fee: RM2,000/year
• Insurance Premium (Not required for those aged 60 and above)
